Heart failure is the result of the heart not being able to pump blood. Heart failure is becoming an increasing burden to Canadians and health care. In recent years, the number of hospital admissions due to heart failure and its costs has increased. Many hospital admissions due to heart failure can be prevented through patient self-management (patient awareness of how they feel). Clinics that help patients with self-management result in less hospital admissions and improved patient quality of life. Patients with heart failure who live in smaller urban and rural areas do not have access to these special heart failure clinics. Therefore, patients with heart failure in smaller urban and rural areas do not receive the same health care.
