Practicum Forms

Instructions:

1. Download and save each required form onto your computer.
2. Open the form Acrobat Reader 8 or above. * Mac users must download and install Adobe Reader. Do not use the built-in 'Preview' PDF reader on a Mac because the data will not be saved properly *
   

   

3. Complete the form(s)
4. Save the completed form(s) to your desktop or a designated folder (please append your last name to the filename)
5. Attach the PDF form(s) to an email and send it to fhs_practicum@sfu.ca (or to the list of addressees indicated on each form) with the subject 'Practicum Forms'
