ACCESS TO THE APPLICATION SYSTEM FOR FALL 2012 OPENS OCTOBER 1ST, 2011

** SFU has a very limited number of major entrance scholarships for exceptional students. For applicants to be considered for entrance awards, complete applications (i.e. the online application and supporting documents) must be received by January 15th. If you wish to be nominated for an entrance scholarship, we strongly encourage you to meet this deadline.

Application Procedures

  1. Complete the application to Graduate Programs on the SFU Dean of Graduate Studies (DGS) website. All applicants must apply online. Note: the To Do List on your SFU Graduate Studies Application will not be updated.
  2. Pay your application fee. The Faculty of Health Sciences will not be able to access your online application until the application fee has been processed. When your application fee has been processed you will be notified by e-mail by the Office of the Dean of Graduate Studies (DGS).
  3. Applicants are advised to complete the online application (#1 above) and payment of application fee (#2 above) by January 15th, to ensure that your application is complete by our deadline of February 1st.
  4. Submit your supporting documents online to the Faculty.
    • Once your application profile is established in the Faculty's admission system, you will receive an email, instructing you to submit supporting documents.
    • Please log on to our admission system as instructed in the email and supply the contact information for your referees (this will initiate the automatic collection of referee reports). You are required to submit the names of 3 referees and their contact information. Mid-career applicants can provide references from up to 2 employers to confirm their readiness for advanced studies.
    • You will then have to submit the required supporting documents, which include the following:
    • A CV or resume that outlines academic achievements and/or awards, publications, relevant work and/or volunteer experience
    • A statement of intent (no more than two typed pages), explaining reasons for pursuing studies in this program, including describing your prior experience in the area of your concentration. If you have already identified topics of special interest for a practicum project or thesis, please include this information in your statement of intent.
    • Electronic copies of your official post secondary transcript(s) are required. In the case of two sided documents we need copies of both sides of your official transcript(s)
    • Copy of TOEFL or IELTS results. In the case of two sided documents we need copies of both sides of your official results.
    • Optional documents. You may submit optional supporting documents; see instructions provided within our online admissions system.
  5. Arrange to have 2 of your official university transcript(s) sent directly by the degree-granting institution(s) that you have attended to:

  6. Proof of English proficiency.
    An applicant whose first language is not English and whose undergraduate degree is from an institution where English is not the language of instruction is required to achieve a satisfactory score on a standardized English test acceptable to Simon Fraser University. For information on acceptable English tests, review the Graduate Studies Admission Requirements.

    IELTS and TOEFL scores need to be sent directly by Educational Testing Service to the Registrar’s office:

    The institutional code required for the TOEFL is 0999.
    • If you have already taken these tests and know your scores, you may report them online to speed up application reviews. Note however that the exam scores must be less than three years old to be considered and the official scores still need to be sent to us for record keeping and verification.
  7. Reminder - Original documents required.
    Originals of your official transcripts and proof of test scores are required before you can be recommended for admission by the Faculty of Health Sciences. Your documents will be verified before you are admitted to the program. Applicants are required to provide us two copies of official transcripts and one copy of official test score results by mail before the admissions deadline.
